This page shows every surface inside CPro360 where a user provides consent to receive SMS messages, together with the exact disclosure language rendered to them. The mockups below are pixel-faithful reproductions of the live product, generated from the same design tokens the app uses. For the interactive live consent form, see /sms-consent.html.
01Account sign-up โ web
The opt-in checkbox is unchecked by default. The user must affirmatively tick it before consent is recorded. The disclosure text immediately adjacent to the checkbox lists the sender, message categories, frequency, opt-out keyword (STOP), help keyword (HELP), and "Msg & data rates may apply", with links to the SMS Program Terms and Privacy Policy.

Existing users can opt in or out at any time from inside the app. The toggle is off by default and shows the same disclosure as the sign-up form. Toggling it on creates a fresh, audited consent record.

After a successful opt-in, the first SMS is the opt-in confirmation. Every subsequent message carries the brand identifier ("CPro360:"), is purely transactional, and includes the STOP/HELP keywords.

Where consent is recorded
Each opt-in capture from either surface above creates a single immutable consent record stamped with:
User id, email, mobile number
Exact disclosure text rendered to the user (versioned)
UTC timestamp (ISO 8601)
Originating IP address and user agent
Capture mode (web_signup, web_settings, or mobile_app)
Records are retained for the life of the account plus 4 years, in line with TCPA recordkeeping guidance.
